Zagreb Medical Studies in English Entrance Exam at a glance
Overview
What is the University of Zagreb medicine in English entrance exam?
The University of Zagreb School of Medicine sets its own entrance examination for its English-taught Medical Studies in English programme. There is no Croatian national medical admission test behind it: Croatia's two English-medium medical schools in our catalogue (Zagreb and Rijeka) both run three-subject science papers of 120 questions with five options each, but they differ on the rules that decide who is eligible, so preparation built around one school's thresholds does not transfer to the other.
The paper is 120 questions, split evenly at 40 each across Biology, Chemistry and Physics. Every question carries five options (A-E) with one correct answer, and results are published as correct answers, per subject out of 40 and in total out of 120. That 40/40/40 split is the single most important structural fact about this exam: Physics is not a minor add-on here, it is exactly one third of the paper and carries its own eligibility threshold, which is a meaningfully different shape from the Bulgarian and Czech schools that test only Biology and Chemistry.
Two separate thresholds apply, and clearing one does not clear the other. You must answer at least 15 of the 40 questions correctly in Biology, in Chemistry and in Physics individually, and you must reach 61 correct answers in total. Clearing all three subject floors at the minimum gives you only 45 correct answers, 16 short of the total, so the two rules are genuinely independent. Admission is capped at 60 students a year, and passing makes you eligible for a conditional Offer of Place rather than guaranteeing one.

Zagreb publishes the rules and sittings for Medical Studies in English on its own pages at mse.mef.unizg.hr, each online sitting with its own application deadline, rather than in one single annual call document. They were read directly on 28 September 2026 and again on 11 October 2026, and still described the 2026/2027 cycle; no 2027/2028 calendar had been issued. Every 2026 sitting was held online. Be wary of figures quoted from the School's separate Croatian-language Medicine admission, which has its own points scale, thresholds and exam day.
Structure
Zagreb Medical Studies in English Entrance Exam format
120 questions across three subjects of exactly 40 each. The Medical Studies in English pages do not state a time limit or whether you may move freely between subjects, so plan your pacing as if the paper were one continuous sitting. The School publishes no weighting by topic or block within a subject, so every question counts as one correct answer towards both thresholds.
| Section | Questions | Weighting | Suggested time |
|---|---|---|---|
| Biology | 40 | 33% (40 of 120); at least 15 correct needed | *see the note below the table |
| Chemistry | 40 | 33% (40 of 120); at least 15 correct needed | *see the note below the table |
| Physics | 40 | 33% (40 of 120); at least 15 correct needed | *see the note below the table |
- No time limit is published on the Medical Studies in English pages
Content
Question and subject breakdown
Three subjects, 40 questions each, each with its own floor of 15 correct answers. Topic scope comes from the School's three official "Requirements for entrance examination" documents, one per subject; those documents carry no date or academic year, and they publish no weighting within a subject, so no block inside Biology, Chemistry or Physics is worth more than another.
Biology
33%Six official blocks: cell biology, reproduction and development, genetics, the human organism, evolution and ecology. In practice that spans cell structure and membrane transport, respiration and photosynthesis, DNA and cell division, Mendelian and molecular genetics, and a substantial run of human anatomy and physiology across the circulatory, respiratory, immune, digestive, endocrine, urinary, nervous, sensory and musculoskeletal systems. The official Biology sample paper contains no figures at all: crosses, karyotypes, base-pairing and physiology items are all posed in text.
Chemistry
33%Three official blocks: general, inorganic, and organic chemistry with biochemistry. One documented gap is worth knowing: reaction rate, catalysis, inhibition and equilibrium are not named anywhere in the official requirements, yet they are tested in the official sample paper, so they have to be prepared regardless. Unlike Biology, the Chemistry paper does use structures and diagrams: Lewis structures, solvent-layer figures, reaction schemes, chirality, and carbohydrate, peptide and nucleobase structures all appear.
Physics
33%A full third of the paper and its own eligibility threshold, the widest-scoped Physics component of any school in our Croatian and Bulgarian catalogue. The official requirements run from units and vectors through mechanics, gases and thermodynamics, waves and acoustics, electricity and magnetism, optics, and atomic and nuclear physics, and they close with a general requirement for simple numerical calculation, basic geometrical formulas and basic physical constants that applies across every block.
Marks
Zagreb Medical Studies in English Entrance Exam scoring
Zagreb's rules for Medical Studies in English are written in correct answers, not points: the Entrance exam page asks for at least 15 correct answers in each subject and 61 in total, and the published results sheets list each candidate's correct answers per subject, out of 40, and in total, out of 120. Be wary of the 5-points-per-task, 600-point scale quoted for Zagreb: it belongs to the School's separate Croatian-language Medicine admission.
- Correct answer
- Counts one towards your subject score and your total
- Incorrect answer
- Not published. The School states only how many correct answers you need and is silent on wrong answers either way: no penalty is likely, but it is not a stated rule.
- Unanswered question
- Not published. Recorded as unknown rather than assumed to be zero.
- Maximum possible score
- 120 correct answers: 40 per subject
- Per-subject threshold
- At least 15 correct of 40 in Biology, in Chemistry and in Physics separately
- Overall threshold
- 61 correct answers of 120
- Reserve questions
- No official source mentions reserve or replacement items, so this is recorded as unknown rather than as none.

The groups below follow the School's three official "Requirements for entrance examination" documents. Where a long official block would have been too coarse to work from (Cell biology, Genetics and Human organism in Biology; General chemistry and the combined Organic chemistry and biochemistry block in Chemistry; Mechanics, Electricity and magnetism and Optics in Physics), it is split along its own bullet groups or paragraph divisions rather than regrouped, so nothing here departs from the official scope.
Six official blocks, split along their own bullet groups. No block carries a published question weighting, and the official Biology sample paper uses no figures at all.
- Cell structure and membrane transport: chemical components of the cell and macromolecules, organization of the prokaryotic and eukaryotic cell, animal and plant cells, cell organelles and their structure, cell membrane structure and transport, endocytosis and exocytosis
- Cell metabolism: cellular respiration, photosynthesis

- Evolution: chemical and biological evolution, human evolution, Darwinian evolution theory
- Ecology: basic ecological terms including environment and abiotic and biotic components, ecosystems, the biosphere, human influence on ecosystems, pollution
Three official blocks, subdivided along their own listed content groups. One area below is derived from the official sample paper rather than from the requirements document; see the note on kinetics and equilibrium.

- Inorganic chemistry: nomenclature of inorganic compounds, trends across the periodic table, representative groups and families including the sulfur, halogen, oxygen and nitrogen families, metals and their compounds, p-block elements and their compounds, transition metals and their compounds
- Organic chemistry: isomerism and stereochemistry, functional groups, basic reaction mechanisms, organic nomenclature, nomenclature, properties and reactivity of alkanes, alkenes, alkynes and aromatic hydrocarbons, halogenoalkanes and halogenoarenes, alcohols, thiols, phenols and ethers, aldehydes and ketones, amines, carboxylic acids and their derivatives, simple five- and six-membered heterocycles
- Biochemistry: carbohydrates in linear and cyclic formulae, monosaccharides, disaccharides and polysaccharides, lipid classification, composition and properties, proteins including basic amino acids and peptide bonds, nucleic acid composition, nucleotides, base pairing, structure and function
The official requirements open with an unheaded block on units, vectors and mathematical functions, then run through mechanics, gases and thermodynamics, waves and acoustics, electricity and magnetism, optics, and atomic and nuclear physics. Their closing "General requirements" paragraph (simple numerical calculation, basic geometrical formulas and basic physical constants) is a skill statement that applies across every block below rather than a topic of its own.

How difficult is the Zagreb Medical Studies in English Entrance Exam?
Zagreb's paper is not conceptually exotic: it sits at strong final-year secondary-school level throughout. What makes it hard is that it asks for three full sciences at once and then checks each of them separately.
Two independent floors, and neither implies the other
You need at least 15 correct of 40 in each of Biology, Chemistry and Physics, and you need 61 correct answers in total. Clearing all three subject floors at the minimum gives you 45 correct answers: 16 short of the total. Going the other way, a candidate with 40 Biology, 40 Chemistry and 0 Physics has 80 correct answers, comfortably over the total, and is still barred by the Physics floor. Neither rule is a shortcut to the other.
Physics is a third of the paper, not a footnote
40 of 120 questions, with its own floor of 15 correct answers, and an official scope that runs from vectors to nuclear binding energy by way of optics and AC circuits. Candidates arriving from Bulgarian or Czech medical-entrance preparation, where several schools test only Biology and Chemistry, routinely underestimate how much of this exam is physics and how far its syllabus reaches.
Figures from the Croatian-language exam do not apply
The School also runs a separate admission for its Croatian-language Medicine programme, marked at 5 points a task out of 600, with a 305-point threshold for 2026/27 and a single exam day on 3 July 2026 from 08:45 to 12:00. None of that belongs to Medical Studies in English, whose own page asks for 15 correct answers per subject and 61 in total. If a Zagreb figure is quoted in points, it is almost certainly the Croatian-language one.
Five options, and no published guessing rule
Every item in the official Biology, Chemistry and Physics sample papers carries five options, which is one more than most schools in our catalogue and makes elimination worth more per question. But the School does not state what a wrong answer costs, or whether a blank is treated differently. No penalty is the likely reading; it is not a published one, so build your exam-day habit around eliminating options rather than around a guessing rule nobody has confirmed.
Few delivery details are published
Every 2026 sitting was an online exam with a rehearsal a few days before, but the Medical Studies in English pages do not say how remote proctoring works, how long the paper lasts or whether a calculator is permitted. Confirm the practicalities with the School before exam day.
Benchmarks
What is a good Zagreb Medical Studies in English Entrance Exam score?
Zagreb publishes more about its thresholds than most schools in our catalogue, and less about everything around them. The two floors and the cap of 60 admitted students a year are stated plainly, and the School's Admission page says the ratio of applicants to enrolled students is usually 4:1. Applicant totals by year and any historical cutoff series are not published anywhere IvyTest has been able to confirm.
Both published floors are counts of correct answers: 15 of 40 in each science and 61 of 120 in total. In the School's published 2026 results it was the total that bound: every candidate marked as failing was below 61, none failed on a single subject alone, and one candidate passed with 11 in Biology. The per-subject rule is still the published one, so build a margin above 15 in all three subjects, and remember that even 20 in each (60 in total) is one answer short of the total. Aim well above the bare pass as well, because passing makes you eligible and places are capped at 60.
Be careful which unit you are working in when you measure progress. Moving from 55% to 65% accuracy across the whole paper is a 10-percentage-point gain in accuracy, which is 12 more correct answers out of 120, taking a candidate from 66 to 78. On a single subject the same 10 points is only 4 answers out of 40, and it is the subject count, not the percentage, that the per-subject floor reads. Quoting a gain in the wrong unit is the fastest way to misjudge how far you actually are from a threshold.
Pending verification
- · Applicant totals by year: the School gives only its usual 4:1 ratio of applicants to enrolled students
- · Historical cutoffs or score distributions by sitting
- · Whether wrong answers carry a penalty, and how blanks are treated
- · The time limit, proctoring arrangements and whether a calculator is permitted
- · Whether the 15-per-subject rule is applied strictly: the official results sheet for 6 July 2026 lists a pass with 11 correct in one subject
These specific numbers aren't public yet — an IvyTest simulator score is the closest benchmark you can get today.

How to prepare for the Zagreb Medical Studies in English Entrance Exam
Three sciences in parallel, because each is checked on its own
Even coverage of Biology, Chemistry and Physics, with Physics given full weight from week one.
- Sit a full 120-question diagnostic in week one and score it four ways: Biology out of 40, Chemistry out of 40, Physics out of 40, and the total out of 120. The per-subject view is the one that predicts eligibility.
- Give Physics the same calendar time as the other two from the start. It is 40 of 120 questions with its own floor of 15 correct answers, and its official scope runs all the way from units and vectors to nuclear binding energy, the photoelectric effect and the de Broglie wavelength.
- Cover the Chemistry kinetics and equilibrium material even though it is missing from the official requirements document: it appears in the official sample paper, so it is examinable regardless of the gap in the published list.
- Add full 120-question timed sittings from week six. The School publishes no time limit, so set a strict one yourself and sit the whole paper without a break: stamina over 120 questions is a problem of its own.
Find the subject nearest its floor and fix that one first
Targeted repair with each subject tracked against 15/40 separately.
- Diagnostic first, then rank the three subjects by distance from their own floor of 15 rather than by overall score: a strong total can hide a subject sitting at 13 or 14.
- Work the weakest subject twice a week and the other two once each, re-testing the weak one against 40 fresh tasks every fortnight.
- One full timed 120-question mock a week from week two, reviewed the same day, with every wrong answer logged by subject and by cause.
- Practise Chemistry and Physics calculations by hand: the School does not state whether a calculator is permitted, so assume not until it says otherwise.
Full-length sittings, every week
Volume of timed papers plus disciplined elimination on five-option items.
- Two full 120-question mocks a week, scored in correct answers per subject and in total, the same units the School's thresholds and results sheets use.
- After each mock, check all four thresholds explicitly: 15 in Biology, 15 in Chemistry, 15 in Physics, and 61 in total.
- Drill elimination on five-option items specifically. With five choices, ruling out two options moves a guess from one-in-five to one-in-three, a larger swing than on the four-option papers most preparation material is built around.
- Rehearse the session shape: all 120 questions in one block with no section breaks to reset between subjects, on the setup you will use for the online exam and its rehearsal.
Protect the threshold you are closest to failing
Triage by per-subject floor, not by overall comfort.
- Stop adding new content. Identify whichever subject sits nearest 15 of 40 in your mocks and spend the majority of your remaining time there: a strong total does not protect a weak single science.
- In Physics, prioritise the high-yield computational blocks that recur across the official requirements: kinematics, Newton's laws, work and energy, fluids, circuits and geometric optics.
- Sit at least two full, scored 120-question mocks in the final ten days, then taper.
- On the day, answer everything you have time for. A penalty for wrong answers is not published and is unlikely, so a blank is the one outcome that is certainly worth nothing.

FAQ
Zagreb Medical Studies in English Entrance Exam frequently asked questions
It is the test the University of Zagreb School of Medicine uses to select candidates for its English-taught Medical Studies in English programme: 120 questions split evenly across Biology, Chemistry and Physics, with five answer options per question. To pass you need at least 15 correct answers in each subject and 61 in total.
No. There is no shared Croatian national medical admission test behind it. Zagreb sets and marks its own paper, and the University of Rijeka, the other English-medium Croatian medical school in our catalogue, sets a separate one. The two happen to agree on 120 questions, three sciences and five options, but they differ on the rules that decide eligibility: Zagreb applies both a per-subject floor (15 of 40) and a total (61 of 120), while Rijeka applies a single pass mark of 60 correct answers and no per-subject minimum at all.
Online, in the 2026/27 cycle. The School ran five online sittings (12 January, 12 May, 8 June, 6 July and 31 August 2026), each with a rehearsal a few days before, and applications had to reach it by post or through an official representative at least 15 days ahead. What it does not publish is the proctoring set-up, the time limit or whether a calculator is permitted, so confirm those directly.
In correct answers: the School's published results sheets list each candidate's correct answers per subject, out of 40, and in total, out of 120. Negative marking is genuinely unpublished: the School states only how many correct answers you need and is silent on wrong answers and blanks in either direction. No penalty is the likely reading, and on that reading a blind five-option guess would add 0.2 of a correct answer on average against 0 for a blank, but since the rule is not published, that is a conditional calculation rather than a fact to build a strategy on.
Two separate thresholds, and both must be cleared. First, at least 15 correct answers out of 40 in Biology, Chemistry and Physics individually. Second, 61 correct answers out of 120 in total. Clearing all three subject floors at the minimum only gets you to 45 correct answers, so the two rules are genuinely independent and neither one implies the other.
Not on the evidence available for this programme. The Medical Studies in English page states 15 per subject and 61 in total, and IvyTest has found no official earlier version with different figures. The moving 305-of-600 and 330-of-600 thresholds often quoted for Zagreb belong to the School's separate Croatian-language Medicine admission, which is marked in points. Re-check the School's page each cycle all the same.
A full third of it: 40 of the 120 questions, with its own floor of 15 correct answers. The official Physics requirements run from units and vectors through mechanics, fluids, thermodynamics, waves and acoustics, electricity and magnetism, optics, and atomic and nuclear physics, and close with a general expectation of simple numerical calculation and basic physical constants. This is the single biggest difference from the Bulgarian and Czech schools that test Biology and Chemistry only.
Yes. The paper is delivered entirely in English for the Medical Studies in English programme, so no Croatian is needed to sit it. Croatian does become relevant during the programme: Croatian Language is a mandatory course from the first year through the third, and the School says it supports the clinical part of the study, where you talk with patients.
Zagreb caps Medical Studies in English at 60 admitted students a year, per its 2026/2027 call for applications. It publishes no acceptance rate as such, but its Admission page says the ratio of applicants to enrolled students is usually 4:1: about four applicants for every student who enrols, the School says. What is also published is the eligibility structure (15 correct in each science plus 61 in total), and clearing it makes you eligible for a conditional Offer of Place rather than admitted.
Within the EU, yes: automatically, as a standard Croatian medical degree, since Croatia is an EU member state and medical qualifications are covered by the professional-qualifications recognition directive. Outside the EU recognition isn't automatic as a rule, but the UK is an exception. Under the Medical Act 1983 (section 21B, as in force on 12 October 2026), a Croatian medical degree is a 'relevant European qualification', so a graduate who applies to the GMC with one, whatever their nationality, is not asked to show the knowledge and skills other international graduates prove through PLAB/UKMLA; evidence of English is still required, and the rule is UK policy that can change. US-bound graduates need ECFMG certification via USMLE Step 1 and Step 2 CK, the same requirement every international medical graduate faces. Croatia-specific in-country licensing steps have not been researched by IvyTest and should be confirmed directly.
Yes. The IvyTest simulator runs Zagreb-format practice (120 questions, five options, across Biology, Chemistry and Physics) in timed, tutor and review modes, with per-subject reporting against the 15/40 floors and the 61-answer total, and a free diagnostic available without payment.
The School publishes no past papers, but its Entrance exam page links three free example tests on its website, one each for Biology, Chemistry and Physics, with 40 five-option questions each, answered online: each answer is marked 'Correct response' or 'Incorrect response' as soon as you choose it. Applicants who have paid the €300 application fee and sent a complete application can also ask for the handbook Entrance Exam Examples with Additional Questions as a PDF: three exams built from 2020 and 2021 questions, with their answers, plus 100 more questions. It may not be passed on.
The 2027/28 sittings are not published yet. In the 2026/27 cycle the School ran five online exams (12 January, 12 May, 8 June, 6 July and 31 August 2026), each with its own deadline: the application had to reach it by post or through an official representative at least 15 days before. Applicants exempt from the exam had to apply by 15 June 2026 and transfer students by 20 June 2026, and enrolment takes place in Zagreb in September.
You need a completed secondary education (normally four years; the School also takes applicants from countries with three-year systems) ending in a school-leaving or state graduation exam, a pass in the entrance exam, a doctor's certificate that you meet the health requirements, and an English certificate unless you are a native speaker or went to an English-medium school (TOEFL iBT 60, IELTS 6 or Duolingo 105, among others). School grades are not used to rank you. A pre-medical college, an MCAT of at least 125 in each section or a BA/BSc in the natural sciences with good credits in Biology, Physics and Chemistry can exempt you from the exam, and a foreign diploma must be recognised in Croatia before you enrol.
Tuition for Medical Studies in English is €12,000 a year if paid at once, or €12,200 in two instalments of €6,100, each paid before the semester begins. Applying costs €300, paid by bank transfer to the School. If you pass and qualify, you accept the conditional Offer of Place by paying a first tuition instalment of €2,500, which is non-refundable. The Letter of Acceptance and the Student Contract follow only once you have paid it and, with your diploma recognised, appear on the joint national list. Housing comes on top: the School quotes private rooms from €200 a month and studios from €300.
Yes, but only into the second year. The School evaluates each transfer once it has all the application documents, including detailed course descriptions and university transcripts, against its own curriculum; for 2026/27, transfer applications were due by 20 June 2026. Study completed at a university abroad must also be recognised in Croatia before you can fully enrol.
The School calls its admission procedure very strict, and its own results sheets bear that out. Of the 120 candidates listed for the 12 May, 8 June, 6 July and 31 August 2026 sittings, 58 passed: 31 of 46, 6 of 18, 13 of 35 and 8 of 21. Every candidate marked as failing was below the total of 61 correct answers, so it was the total, not a single subject, that decided them: every published fail fell below 61. The questions themselves are high-school level, but in three full sciences.
Medical Studies in English lasts six years (12 semesters), carries 360 ECTS credits and leads to the title Doctor of Medicine (MD). The first year covers Anatomy, Medical and Molecular Biology, Physics and Biophysics, and Medical Chemistry and Biochemistry, and Croatian Language is a mandatory course from the first year through the third. Teaching uses problem-based learning and small groups, the final year includes Clinical Rotations in Croatia or abroad, and you finish with a graduate thesis. Each year's schedule and textbook list is published.
On the pages IvyTest read, its Medical Studies in English site quotes no ranking, and IvyTest has not checked any ranking table. What the School does state: it is the oldest medical school in Croatia, founded in 1917 within a university founded in 1669; it educates more than 60% of the country's doctors; Medical Studies in English received the CeQuint certificate for quality in internationalisation in 2015; and 466 students graduated from the programme between 2009 and 2025.
